All NorthstarMLS® forms are now available through the Instanet Forms application. If you need a print copy of a form, follow this procedure:

Click the Matrix LOGIN button on the right side of this screen and log in to Matrix.
On the Matrix Home page, click the "Instanet Forms" link in the "External Links" box (or from the Home tab drop-down menu). A new window will open with the Instanet application.
Click the Forms link (or button).
Select the Form you wish to print from the Form list.
Click on the Print Blank Form button.

Completed Change Forms must be faxed to your Association (Not NorthstarMLS)

This form is to be filled out, signed and returned to RMLS by new brokers to designate assignment of copyright for protection of data.

Instructions for Broker:

On Page 2 of the form, select Option I or Option II. If you do not make a selection, you will automatically be opted in to Option I.

On page 5, fill out all blanks including Designated Broker and Office Number. If you do not fill in these fields, we may not be able to tell who the form is from and therefore not give you credit for returning it.

Return all five pages of the form to NorthstarMLS via mail or FAX (see below). If your MLS access has been turned off, please fax the completed agreement. We cannot turn your access back on until we receive your completed and signed agreement.
