MLS Forms
All NorthstarMLS® forms are now available through the WEBForms™ application. If you need a print copy of a form, follow this procedure:
- Click the Matrix LOGIN button on the right side of this screen and log in to Matrix.
- On the Matrix Home page, click the "WEBForms" link in the "External Links" box. A new window will open with the WEBForms application.
- On the right side of the WEBForms screen, click the "Printable Forms Library" tab.
- Click the "View/Print" link next to the name of the form you want. (For the NorthstarMLS forms, scroll down the list to the ones that start with "NSMLS.")
- Type in an MLS number if you want to auto-fill data from the MLS. Otherwise, leave that blank and click the "View/Print" button.
- If you need help, contact our Help Desk at 651-251-5456 (Monday through Friday, 8:00 a.m. to 5:00 p.m.) or help@northstarmls.com.

Broker Participation Agreement
This form is to be filled out, signed and returned to RMLS by new brokers to designate assignment of copyright for protection of data.

Instructions for Broker:
- Print and read the form.
- On Page 2 of the form, select Option I or Option II. If you do not make a selection, you will automatically be opted in to Option I.
- On page 5, fill out all blanks including Designated Broker and Office Number. If you do not fill in these fields, we may not be able to tell who the form is from and therefore not give you credit for returning it.
- The Broker must sign the form. Be sure to print your name under your signature.
- Return all five pages of the form to NorthstarMLS via mail or FAX (see below). If your MLS access has been turned off, please fax the completed agreement. We cannot turn your access back on until we receive your completed and signed agreement.
